# Operating discipline for people who run clinics

A practical decision method for an Operations Head: separate responsibilities, record assumptions and act on the work within your control.

An Operations Head needs a way to separate a change in the market from a decision the team can make today. I use two questions: what is within our responsibility, and what action follows from the part that is not? The distinction is especially useful when an ambitious plan for medical tourism in India meets the daily work of running clinical services.

## Write the two lists

On the first list I would put the service commitments we make, the people assigned to them and the evidence we collect. On the second I would put exchange rates, travel restrictions, competitors' choices and the patient's independent decision.

The second list still deserves attention. Each item needs an owner to monitor it and a condition that would trigger a response. What it does not need is a meeting that repeatedly describes the same uncertainty without deciding anything.

## State the condition on the commitment

A launch date is an operational commitment with dependencies. If clinical staffing, required approvals or support coverage is incomplete, the date needs to move. I would write those conditions when approving the plan.

For an aesthetic medical travel programme in India, this means defining readiness before accepting the first overseas booking. A clinician must own clinical readiness. The Operations Head must know whether records, escalation, travel coordination and the agreed follow-up service are ready to support that care.

## Use a decision record

My decision record has four entries: the choice, the expected effect, the evidence that would challenge it and the date for review. It should be short enough to complete while the reasoning is fresh.

At review, I would compare the original expectation with what happened. A good outcome does not prove that the reasoning was sound, and an unwelcome outcome does not automatically prove it was careless. The useful question is which assumption needs to change.

> A commitment becomes useful when its owner and its conditions are visible.

## Rehearse the difficult version

Before expanding, I would ask the team to work through a delayed arrival, an unavailable coordinator and a patient who needs further clinical assessment after returning home. The exercise should end with named actions and unresolved gaps.

A rehearsal is also a test of authority. Who can authorise extra support? Who can change the schedule? Who is qualified to make the clinical decision? A plan with many names but no clear decision rights is not ready.

## Give the ambition an operating form

I want India to be a destination patients can trust for aesthetic care. As an operating ambition, that requires more than a confident forecast. It requires honest eligibility decisions, reliable records and a response when the patient's experience does not match the plan.

I would review those commitments at the same table as growth. The purpose is to build a service that can sustain a reputation, with evidence available before anyone asks for it.

India's medical tourism market will grow at its own pace, and each patient will make their own choice. The discipline sits in three decisions: which promises we make, what proves we kept them and when an unmet condition stops the next booking. That is what I would bring to the role.

## Questions people ask

### How should an Operations Head handle uncertainty?

Separate the work the organisation controls from external events it needs to monitor. Give each material external risk an owner and a response trigger. Then concentrate daily management on decisions, staffing, standards and evidence within the organisation's authority. This keeps uncertainty visible without allowing it to consume every operational review.

### What belongs in an operations decision record?

Record the decision, the effect expected, the evidence that would challenge it and a review date. Include the assumptions that materially affect the result. The record should make later learning possible without relying on memory or judging the original decision solely by whether the eventual outcome happened to be favourable.

### How does operational discipline support medical tourism in India?

It turns an international care promise into work with owners, evidence and conditions for proceeding. For aesthetic treatment, that includes clinical assessment by qualified professionals, clear charges, appropriate records and planned follow-up. India's reputation can be strengthened by providers that consistently demonstrate those commitments and respond openly when the plan changes.
